To enroll in viscosity testing for your practice, print and fill out this Clinician Enrollment Form, then fax it to us at 610-524-9440.

Payment with specimen is required. We accept checks, money orders, Visa or Mastercard. We do not accept American Express or Discover.

We do not accept Medicare assignment as payment, and we are not able to process insurance reimbursement claims at this time.
